I would like to request for a copy of my income tax return for the year 2021 and my quarterly income tax return for the 1st and 2nd quarter of 2022

I would use the ITR as proof of income as I am applying for a credit card.

Greetings!

Thank you for your request dated Oct 21, 2022 06:38:02 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Income Tax Return.

Response to Request

Per our telephone conversation today, October 21, you were informed that if you need a Certified True Copy (CTC) of your Income Tax Return (ITR) for the year 2021 and Quarterly ITR for the year2022, you may personally or through your authorized representative file your request to the Revenue District Office (RDO) No. 56-Calamba Laguna and look for Mr.  Bayani Atienza, kindly bring the Original Return and a Photocopy of your ITR. 

Any request for certification that may be requested by a taxpayer from the RDO on matters relating to its registration shall be charged with a fee in an amount not exceeding one hundred pesos (P100.00), in addition to the documentary stamp tax imposed under Section 188 of the National Internal Revenue Code, as amended.
