Dear Mr. Mendez:

I refer to my letter of Mar 03, 2023 03:14:55 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, request for a copy of your Income Tax Return and/or BIR Form 2316.

We called your mobile number today, March 3, to validate your existing eFOI request, however, if was unsuccessful.

You have indicated in your request that you require a copy of your Income Tax Return (ITR) for Taxable Period 2021. For verification and clarification, kindly provide additional information on the following:

1. Did you have a registered business between the period 2021? Or were you an Individual Earning on Purely Compensation Income from the period 2021?

2. If employed, kindly provide the Complete Name of your Employer, Employer's TIN and Address of your Employer?

3. Kindly provide your TIN Number and Middle Name.
