Dear Mr. Patlin:

I refer to my letter of Sep 28, 2022 04:58:36 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return (ITR) and/or BIR Form 2316.

In reply, kindly provide us the following data in order for us to evaluate your requested information:
1. Your Tax Identification Number (TIN);
2. Name of your Employer/s (for Calendar Year 2020-2021);
3. TIN of your Employer/s; and
4. Address of your Employer/s.

Email the needed information at sherry.ann.arcita@bir.gov.ph as this contains sensitive information and indicate in the subject email tracking number #BIR-060712813469, to respond.

We also called your contact number, sadly it was unsuccessful, to confirm if you have ended your employment this year 2022, it is still the responsibility of your EMPLOYER to furnish you a copy of  BIR Form 2316 on or before January 31 of the succeeding calendar year, or if the employment is terminated before the close of such calendar year, on the day of which the last payment of compensation is made. 

The Revenue District Office (RDO) gets a hold of the said BIR Form 2316 upon submission of your employer on or before February 28 (per RR 2-2015 as amended by RR 16-2021) of the following year or in your case on or before February 28, 2023.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Dec 23, 2022 03:23:54 AM, your request will be closed.
