Would like to request for certified true copies of my BIR Forms 2316 for the years 2012 to 2021.
These will serve as my personal copies.

Dear Mr. Lorayes,
I refer to my letter of Mar 24, 2022 09:45:53 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Certified True Copy of BIR Form 2316.
In order for us to properly endorse and work further on your request, kindly provide us the TIN, RDO and complete name and address of your employer (from 2012-2021)
On the other hand, it is also advised that you better ask for a copy of your 2316 directly from your employer as they are the main source of such documents. It is also mandated by law that every employer shall issue BIR Form No. 2316 to every employee before the close of such calendar year, or if employment is terminated, on the day on which the last payment of compensation is made.
We would also like to suggest that if you know where you filed your 2316, you may directly request it to the concerned RDO, bring the duplicate/xerox copy with you for they may also need to validate and determine if you are the rightful owner of the documents requested as these are sensitive information and are bound by the Data Privacy Act of 2012 and Sec 270 of the NIRC.
During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jun 16, 2022 02:07:14 AM, your request will be closed.
