Good day sir, I, Ellen Tarrre Aguirre, a midwife II, a filipino citizen, of legal age, married with postal address 3 magtolis st. Brgy. 3 Sipalay City Negros Occidental, presently working at Merceditas J Montilla District Hospital, hereby formally requests for my ITR 2021. Hoping for your kind consideration. THank you.

Greetings!

Thank you for your request dated Dec 02, 2021 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your request
You asked for 2021 ITR.

Response to your request
While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested as your ITR for 2021 is yet to be filed on or before April 15 next year, hence your request is denied.

However, Ms. Ellen Aguirre may just visit the Revenue District Office where she filed her ITR. submit a request herself and attach scanned copies of at least two (2) valid government I.Ds containing her birthdate, photograph and signature for the frontline officers of the bureau may also need to validate and determine if the requestor is the rightful owner of the documents requested as these are sensitive information and are bound by the Data Privacy Act of 2012 and Sec 270 of the NIRC.
