Dear Mr. Tuddao:

I refer to my letter of May 10, 2023 06:46:12 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return and/or BIR Form 2316.

In reply, kindly provide the following;

1. Name of your Employer for Calendar Year 2020;
2. TIN of Employer;
3. Registered Address of your Employer;
4. Your Birthday;
5. Your Address;
6. Your TIN; and
7. Your contact number (mobile no.) in case we need to call you for further information.

Email the needed information at sherry.ann.arcita@bir.gov.ph and indicate in the email subject ticket number #BIR-083009086205, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Aug 03, 2023 12:23:01 AM, your request will be closed.
