Hi! Im Mark Wendell P. Hije, I would like to request for a copy of my Income Tax Return During June 2016 to December 2017, My previous employer is Golden Arches Development Corporation. This request will be needed for my present job. My Tin Number is : 330300145. I am currently residing in province thats why I cant go to the office due to this pandemic.

I refer to my letter of Oct 09, 2020 04:37:50 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return for the Year June 2016- December 2017.
In order for us to work further on your request, Kindly email us the complete business name and address of your employer from June 2016 – December 2017 and its/their TIN for us to be able to properly endorse your request to the concerned Revenue District Office. Furthermore, pursuant to the requirements indicated in our FOI Manual, requester should attach two valid IDs with birthdate and photograph, for validation purposes.
During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jan 04, 2021 03:35:11 AM, your request will be closed.
