I refer to my letter of May 30, 2022 09:38:39 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR Form 2316.

You have indicated in your request that you require a copy of your ITR for taxable periods 01/01/2019 - 12/30/2019. For verification and clarification, kindly provide additional information on the following:

1. Did you have a registered business between the period 01/01/2019 - 12/30/2019? Or were you an individual earning on purely compensation income from the period of 01/01/2019 - 12/30/2019?

2.  If employed, kindly provide the name of your employer and registered address of your Employer for 01/01/2019 - 12/30/2019?

3. Withstanding your answer to the first and second query, did you file an ITR for the cited periods?

In relation to your request for a CERTIFIED TRUE COPY of BIR FORM 2316 for taxable year 2019, we would like to ask for the original copy of BIR FORM 2316 given to you by your employer in order for us to verify if it is the same as the one on our records, 

With this in hand and your valid identification card, you may proceed directly to our Office at Along Bypass Road, Barangay Tiaong, Guiguinto, Bulacan for us to process your request.  A Certification Fee of P100 is required per copy.
