Hi, I would like to request another copy of my ITR. My employer issued a copy for me but I wasn't able to get it because the main company was farther away from where I live and I am pregnant as of the moment so I was restricted of traveling. 

Kindly accept this request for I am really in need of this document. Thank you so much!

I refer to my letter of Jun 07, 2023 12:52:14 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return and/or BIR Form No. 2316 for credit card application.

Relative to this, kindly provide the following;

> Name of Employer for the Calendar Year 2022;
> TIN of Employer (as this will be endorsed to the concerned Revenue District 
   Office, where your employer is registered);
> Registered Address of your Employer;
> Your Date of Birth;
> Your Address; and
> Your TIN.

Kindly email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the subject email ticket number #BIR-098000994531, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Aug 29, 2023 11:54:48 PM, your request will be closed.
