1. if you are employed/employee (income derived from compensation from your employer) or self-employed (with business and/or practicing one's profession and employed at the same time from that year/period?
2. if Employed: State Employer's Name and Address and Tin (if applicable);
3. if Self-employed: Business Name, Address and Tin.
