Dear Ms. Banay:

I refer to my letter of Feb 27, 2023 02:46:50 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR Form 2316 (Bella Chica Beauty Studio).

Kindly asked your former employer/agency if they already submitted to the Revenue District Office (RDO) where they are registered Certified List of Employees Qualified for Substituted Filing of ITR (Annex F) of Revenue Regulations No. 11-2018 (Notarized) together with scanned copies BIR Form 2316 saved in DVD-R format or USB memory stick.

The submission of the abovementioned document is on February 28, 2023. Just like in case, if your former employer/agency have not yet submitted the Annex F together with the scanned copies of BIR Form 2316 of their employees, the RDO would not be able to give you a copy.

In view of the foregoing, we recommend that you request your BIR Form No. 2316 directly from your former employer/agency, as this should have been issued to you on or before Jan. 31 of the succeeding year, or on the day of last payment of wages in case of termination from employment.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before May 22, 2023 08:15:31 AM, your request will be closed.
