2. Or Self Employed Individual (With Business, Practicing One's Profession or Under Job Order)?
3. What Income Tax Return (ITR) are you referring to - BIR Form 2316 for Employees or BIR Forms 1701 & 1701A for Self-Employed Individuals?
4. Name of your Employer last 2020-2021?
5. Employer's Business Address; and
6. Employer's Business TIN (if applicable).
Kindly email the needed information at sherry.ann.arcita@bir.gov.ph as this contains sensitive information and for us to properly endorse your request to the concerned Revenue District Office (RDO). Indicate in the email subject Tracking Number #BIR-124026489172, to respond.
