In reply, if you want a stamped BIR Form No. 1905 you may proceed with Revenue District Office (RDO) No. 28-Novaliches with office address located at 2/F to 5/F BDI Bldg., Lot 35, Blk 167 Commonwealth Avenue, Brgy North Fairview, Quezon City.

You may get in touch with the Client Support Section (CSS) of RDO 28 at (02) 8929-2202 and (02) 8929-7981. You may also send your duly accomplished BIR Form No. 1905 and scanned copies of your two (2) valid government issued I.Ds at rdo_28css@bir.gov.ph
