Good Day!

I am writing this email on behalf of my brother, Lloyd Anthony R. Ocampo. He is currently working as a Nurse NDP II at our municipality in Bambang, Nueva Vizcaya under the Department of Health Region 2. We would like to request soft copies of his ITR/BIR 2316 for the years 2019, 2020, and 2021. Thank you!

ii.	copy of one (primary) or two(secondary) government-issued IDs bearing the signature of the requesting party iii.	original one (1) primary or two (2) secondary government-issued IDs of the representative.
However, you may just advise your brother to submit a request himself and attach scanned copies of at least two (2) valid government I.Ds containing his birthdate, photograph and signature or just visit the Revenue District Office where he filed his ITR.

For your guidance, please refer to the Revised People's Freedom of Information Manual for the Bureau of Internal Revenue, which is available for download at the Bureau's Official Website or www.bir.gov.ph.

Thank you.
