June 4, 2022 Dear Mr. Mark De Leon: Greetings! Thank you for your request dated Jun 03, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You request for Tin # with 1904. After careful consideration, we resolve to deny your request due to the reason that the request falls outside the scope of the FOI program (i.e. the request is a front-line/government service of the agency, etc.). The BIR Form 1904 is downloadable at the BIR Website at https://www.bir.gov.ph/index.php/bir-forms.html.
