Hi. May I request for the copies of my BIR form 2316 for the years 2020-2022. If the forms are not available, I would like to request for a certificate of compensation and withholding tax information which  can be found in the Alphalist of Employees filed by my employer for the mentioned years. Thanks!

I refer to my letter of Aug 28, 2022 01:53:50 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Form 2316.

Kindly provide us the following data in order for us to evaluate your requested information:
1. Your Tax Identification Number (TIN)
2. Name of your Employer/s
3. TIN of your Employer/s
4. Address of your Employer/s

You can also ask from your employer copy of the Duly received Annex F (Proof that your employer came to Revenue District Office) and have the List & BIR Form 2316 of their employees who availed of substituted filing. Then bring a copy of 2316 of the requesting party for stamping of certified copy.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Nov 22, 2022 05:54:09 AM, your request will be closed.
