I refer to my letter of Sep 14, 2023 11:59:18 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return.

If the request is made through a representative, please be guided of the following:

1. In addition to the scanned copies of at least two (2) valid government I.D.s containing the photograph and signature of the requesting party/principal (Mr. Yap);
2. We need to have the scanned or soft copies of the written authorization and/or Special Power of Attorney (SPA) signed by the principal; and
3. Scanned copies of at least two (2) valid government I.D.s containing the photograph and signature of the representative (per Section VII-Standard Procedure for eFOI Requests, Section 21.3 (a) (b).

Kindly email the requested document at sherry.ann.arcita@bir.gov.ph and indicate in the subject email ticket number #BIR-150827144903, to respond.
