Greetings!

Thank you for your request dated Jun 22, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You request for a Tin verification slip.

After careful consideration, we resolve to deny your request due to the reason that the request falls outside the scope of the FOI program (i.e. the request is a front-line/government service of the agency, etc.).

We would like to advise you to visit the nearest Revenue District Office (RDO) on your request. Please bring two (2) valid government-issued I.Ds containing the photograph and signature as proof of your identity. If the request is made through representative, a written authorization and/or special power of attorney of the requesting party (principal) and at least two (2) valid government IDs containing the photograph and signature of the representative shall be presented.
