I refer to my letter of Aug 08, 2022 07:18:58 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return / BIR FORM 2316.

We been calling your contact number today August 10, 2022 but it was unattended/unsuccessful, to inquire/verify/further validate the name of your employers for 2014-2015 as when we checked them with the BIR system it does not match the information.

Therefore, kindly give us the Correct Name of your Employers for 2014 and 2015 and Taxpayer Identification Number (TIN) (if applicable) and email them at sherry.ann.arcita@bir.gov.ph as this contains sensitive information. Kindly indicate in the email subject tracking number #BIR-174432925745, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Nov 02, 2022 01:27:16 AM, your request will be closed.
