Greetings!
May I request for a copy of my Income Tax Return for the period of 2017 and 2018. I need my previous ITR because I lost my copy when I resign from CHED last 2019. I was just hired as permanent employee at government agency thus i need to process my BIR form 1901 but the BIR RDO 039 required me to closed my professional registration and change it to local employment. My TIN was 314-836-410.

5. Employer's Business Address; and
6. Employer's Business TIN (if applicable).
