After careful consideration, please submit via email your Duly Accomplished BIR Form No. 1905 (which is downloadable at the BIR Website https://www.bir.gov.ph) and attach any Government Issued I.D.s to the Revenue District Office (RDO) where your TIN is registered. In your case, you are registered at RDO 50-South Makati (Not in RDO 126)  and their email address is rdo_50css@bir.gov.ph.
