Greetings!

Thank you for your request dated May 30, 2022 06:45:14 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You Request for your BIR Form No. 2316 for Calendar Year 2019.

In reply, we already called your contact information today (May 30, 2022) to apprised you that the issuance of your BIR Form 2316 should be with your employer being the one and the main source of the document.

As an alternative, you are scheduled to visit our Revenue District Office in Pampanga on Wednesday (June 1, 2022) which is located in BIR Complex, Sindalan City of San Fernando Pampanga. You are advised to bring your request letter and valid ids.
