September 27, 2022 Dear Mr. Macabagdal, Greetings! Thank you for your request dated Sep 23, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for BIR FORMS - 2316, 1905, 2305 & TIN VERIFICATION SLIP. Response to your request After careful consideration, we resolve to deny your request due to the reason that the request falls outside the scope of the FOI program (i.e. the request is a front-line/government service of the agency, etc.). We would like to advise you to visit the nearest BIR Revenue District Office (RDO) on your request. Please bring two (2) valid government-issued I.D.s containing the photograph and signature as proof of your identity for validation purpose. Addresses and contact numbers of RDOs can be found in the BIR Website at www.bir.gov.ph homepage/contact us/directory/revenue district offices links. In addition, the BIR Form 2316 must be accomplished by an employer for each employee receiving salaries, wages, and other forms of remuneration, indicating the total amount paid and the taxes withheld for the calendar year. It must be issued to the employee on or before Jan. 31 of the succeeding year, or on the day of last payment of wages in case of termination from employment. This certificate, will serve as your ITR being an employee earning purely compensation income regardless of the amount. You are qualified to “Substituted Filing” of ITR pursuant to Revenue Regulation (RR) No. 2-98 as amended. If the request is made through representative, a Special Power of Attorney of the requesting party (principal) and at least two (2) valid government IDs containing the photograph and signature of the representative and the principal shall be presented. Please be reminded that TIN is considered a piece of sensitive personal information that is bounded by the Data Privacy Act of 2012. Thank you.
