Greetings!

Thank you for your request dated Jun 16, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You request for a 1902 Form.

While our aim is to provide information whenever possible, our office does not have the document or copy of your BIR Form 1902. Instead kindly secure  for a TIN Veriifcation Slip at the Revenue District Office (RDO) as proof of your Taxpayer Identification Number (TIN).
