I refer to my letter of Aug 23, 2022 06:45:34 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR Form 2316.

Per our phone conversation this morning (August 23, 2022) kindly provide us the Name of your Employer for Taxable Year 2022, Employer's Business Address and Employer's TIN for us to coordinate your request/concern to the district office where your employer is registered and where they submit their reports.

 Email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the email subject tracking number #BIR-210154128910, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Nov 15, 2022 03:03:25 AM, your request will be closed.

We hope to hear from you at the soonest possible time.
