Dear Ms. Calam:

I refer to my letter of May 17, 2023 07:36:25 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return.

In order for us to work further on your request, we need more information from you regarding your query.  In particular, it would be helpful to know the following details:

1)  What Income Tax Return did you file and are requesting from us? Is it BIR 
     Form 1701 or BIR Form 1701A?
2) What is the Complete name indicated in the Income Tax Return you filed?
3) What is your Tax Identification Number indicated in your filed return?
4) What Revenue District Office (RDO) did you file your BIR Form 1701 or 1701A?
5) Did you file Online (eBIRForm Package)  or over the counter?
6) Date/Month/Year of Filing of the BIR Form 1701 or BIR Form 1701A?

Emailed the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the subject email  ticket number #BIR-223606295218, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Aug 09, 2023 01:02:40 AM, your request will be closed.
