To whom it may concern, 

I was given a confirmation message by my employer that my name has a similar record in the BIR database which may have been previously registered with the BIR, and I was asked to proceed to the nearest RDO.

Could you please let me know what appropriate BIR forms and documentary requirements I will need to bring for further verification? 

I have contacted BIR 2 weeks ago, however I did not receive any response. Looking forward to your reply, thank you.

We were able to determine that your request cannot be counted as a valid FOI request as this is part of the frontline service of the BIR, hence your request is denied.

In this regard, we would like to advise you to visit the concerned BIR district and bring with you the email the notification  that you have similar record and the BIR Form 1902 and the documentary requirements needed for the Processing of your Application like Birth Certificate and your valid ID
