Hi I would like to request for Certificate of Non-filing of Income Tax Return for the scholarship application requirements of my daughter in National University.

Greetings!

Thank you for your request dated Nov 15, 2023 11:12:53 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Affidavit of Non-filling Tax.

Response to Request

This is in connection with your request for information for the Requirements for Scholarship Application.

After careful consideration, kindly advised your parents to accomplish and submit the Checklist of Documentary Requirements (For those Persons with Low Income/No Income) who may want to avail of the Scholarship and Job/Livelihood Programs:

1.	Certification of Low Income/No Income signed by the Barangay Chairman of the place where the applicant resides;
2.	Duly Notarized Affidavit of Low Income/No Income from the Notary Public;
3.	Request Letter;
4.	Photocopy of Identification Card with three (3) specimen signature;
5.	Payment of Certification Fee (P100.00) and Loose Documentary Stamp Tax (P30.00), per copy;
6.	If transacting through a representative, a Special Power of Attorney (SPA) shall be executed by the taxpayer-applicant and Any government-issued IDs of the Taxpayer and Authorized Representative.

Submit the above mentioned documents/requirements at the Collection Section of the
Revenue District Office (RDO) having jurisdiction over the place of Residence of the Taxpayer.
