May 10, 2022 Dear Ms. Mimi Norizuki: Greetings! Thank you for your request dated May 06, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. You request for your INCOME TAX RETURN (ITR) for 2021 & 2022. In response to your request, we have called you on your contact details to verify some information. And since you are registered as Professional and confirming your current status as a Job Order Contract under Department of Health (DOH), you should have filed your ITR Form/s - BIR Form 1701 or BIR Form 1701A (whichever is applicable) on or before April 18, 2022. You also informed us that you have filed your ITR Form through eBIRForms Package and have paid your corresponding tax due.
