1. Are you requesting for your Income Tax Return for the Year 2018?
2. Are you a Compensation Income Earner or Local Employee for the Year 2018?  
3. Or Self Employed Individual (With Business, Practicing One's Profession or Under Job Order)?
3. What Income Tax Return (ITR) are you referring to - BIR Form 2316 for Local Employees or BIR Forms 1701 & 1701A for Self-Employed Individuals?
4. Name of your Employer last 2018?
5. Employer's Business Address; and 
6. Employer's Business TIN (if applicable).
