1. Lacks of Valid Proof of Identification Card; and
2. For verification purposes of your existing TIN Number, We would like to advise you to visit the nearest BIR Revenue District Office (RDO) on your request. Please bring two (2) valid government-issued I.D.s containing the photograph and signature as proof of your identity for validation purpose.
