Hi,

I would like to request a copy of my BIR 2316 from the years 2013 and 2017 since I've lost the copies given by my previous employers. I need these for VISA application as proof of employment for James Noah Bungag.

My TIN is 314-792-590-0000.

My Employer for 2013-2014 is ACCENTURE, INC. with TIN 000-845-543-000. The employer doesn't have copies anymore.

My Employer for 2017 is Acestar Solutions and Trading, Incorporated with TIN 004-598-112-0000. I've reached out to the employer and they claim not to have copies anymore since the company dissolved in 2019.

Greetings!

This is with regard to your request for copy  of BIR 2316 dated August 5, 2022 sent through the eFOI portal.  The office regrets to inform you that we cannot furnish the requested BIR Form 2316.  Upon checking the Alphalist of Employees submitted electronically by Acestar Solutions and Trading company for the taxable year 2017, it was found out that your name was not included in the list of employees.
