Dear Ms. Granada,

I refer to my letter of Jul 09, 2021, 02:11:43 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for a Copy of BIR form 2316.

In order for us to work further on your request, we need more information from you regarding your query. Please provide us your employer's TIN, business name & address,  and email it @ ana.marie.du@bir.gov.ph, for us to properly endorse your request to the concerned RDO.

However, you may also ask for this form from your employer as it is mandated by law that every employer shall issue BIR Form No. 2316 to every employee before the close of such calendar year, or if employment is terminated, on the day on which the last payment of compensation is made.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Oct. 01, 2021, 12:14:15 PM, your request will be closed.
