I would like to request a copy of my tax income return from year 2016 to year 2023.

My Maiden name is : alyssa symerz s. milan
My Married name is: alyssa symerz m. teruel

Is it possible that it is a certified copy?

Dear Ms. TERUEL:

I refer to my letter of Jun 28, 2023 06:45:39 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request of copy of ITR (BIR Form 2316) from year 2016 to 2023.

In reply, please be informed that your request for a copies of your BIR Form No. 2316 for CY 2016 to 2023 can be obtained through your EMPLOYERs as they are obligated to furnish every employee a copy of the aforesaid form as a proof of your taxes withheld which shall serve as your Annual Income Tax Return  according to Revenue Regulation 2-98, as amended.

The employer shall prepare BIR Form No. 2316 in triplicate, which shall be distributed as follows:

(1)	Original – Employee’s copy;
(2)	Duplicate – BIR’s copy; and
(3)	Triplicate- Employer’s copy which shall be retained for a period of ten (10) years.”

Kindly coordinate with your employers if you have not received a copy of your BIR Form No. 2316 for CY 2016 to 2023.

If you need a Certified True Copy (CTC) of the document, you may personally or through your authorized representative file your request to the Revenue District Office (RDO) where your EMPLOYERs are registered.

Kindly bring the Original and Photocopy of your BIR Form 2316 and you may also ask from your employer copy of the Duly Received “Annex F”. 

Any request for certification that may be requested by a taxpayer from the RDO on matters relating to its registration shall be charged with a fee in an amount not exceeding one hundred pesos (P100.00), in addition to the documentary stamp tax of Thirty Pesos (P30.00) imposed under Section 188 of the National Internal Revenue Code, as amended.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sep 22, 2023 12:26:00 AM, your request will be closed.
