I would like to request your good office for a Certified True Copy of my ITR (BIR Form 1700) filed for the calendar year 2021.  The said copy will be used as supplemental documents for the enrolment of my child.

I refer to my letter of Jun 14, 2022 09:54:39 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return - 2021.

In relation to your request for a Certified True Copy of your Annual Income Tax Return (BIR Form 1700) for taxable year 2021, please provide us the following:

1. When did you file your Annual Income Tax Return (BIR Form 1700)?
2. Where did you file your Annual Income Tax Return (Manually at the Revenue District Office or through eBIRForms)?
3. Employer's Name, Business Address and Taxpayer Identification Number (TIN)?
4. Requestor's TIN Number?
5. Copy of the BIR Form 1700 (if applicable), it would be helpful to know this information in order to proceed with your request. Kindly email the needed information at sherry.ann.arcita@bir.gov.ph as this contains sensitive information. Indicate in the subject email the ticket number #BIR-277933335428, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Sep 07, 2022 04:27:50 AM, your request will be closed.
