I would like to make a request for a copy of Form 2316 for the year 2021. I recently ended my contract in Child Jesus of Prague School, a private school, located in Binangonan Rizal. I regrettably lost my copy provided by my lawyer. I sincerely apologize for that.

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested as this is yet to be filed next year. You may try to ask your previous employer for a copy of BIR Form No. 2316 (Certificate of Compensation Payment/Tax Withheld) as they may still have a copy of the same.
