Hello po,

Mag request po sana kami ng copies ng 2316/Alphalist received po ng RDO 52. Yung manager po namen sa current company ko, ay nirerequire ni POEA ng copy as part po ng documents para sa among license application.

Yung previous company po ng manager ko nagsara na kaya hindi na po kami makahingi ng copy sa kanila.

Ano po kayang pwede nameng gawin para makarequest ng copy.

I refer to my letter of Jan 16, 2024 06:19:39 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for 2316 or copy of received alphalist for CYs 2021 and 2022.

In reply, kindly provide us the following information:

1. Name of your Employer's for CY 2021 and 2022;
2. TIN of your Employer's; 
3. Employer's business address;
4. Your Complete Name (including Middle Name);
5. Your Date of Birth; and
6. Your TIN Number.
