Hi Ma'am/Sir, 

Good day!

May I humbly request a copy of my ITR Form 2316 from my previous employer at Artemis Express Inc. their registeres business is at Lot 86 Bagsakan rd., corner Avocado st., FTI Complex, Taguig City, Philippines. I was employed by the said company from 2019 to 2021.

I am having difficulties of requesting the said document due to company reasoned out it was 2 years ago already.

Hoping for positive response on this matter.
