Greetings!

Thank you for your request dated Jul 29, 2022 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You request for a TIN VERIFICATION SLIP.

In reply, you may visit our Revenue District Office (RDO) located at Export Bank Plaza cor. Chino Roces and Sen. Gil Puyat Avenue, Makati City on your request for TIN Verification. 

Please bring two (2) valid government-issued I.Ds containing the photograph and signature as proof of your identity. If the request is made through representative, a written authorization and/or special power of attorney of the requesting party (principal) and at least two (2) valid government IDs containing the photograph and signature of the representative shall be presented.

Please be reminded that TIN is confidential information that is bounded by the Data Privacy Act of 2012.
