Hi,

I hope this letter finds you well. I am writing to request a copy of my Income Tax Return (ITR) for the financial year 2022-2023.

My name is John Paul Baloyo Montilla, and my tax identification number is 470161849. I am in need of a copy of my ITR for my tourist visa application.

I would be grateful if you could provide me with a copy of my ITR at the earliest convenience. 

Thank you for your assistance in this matter. I look forward to your prompt response.

Dear Mr. Montilla:

I refer to my letter of Aug 02, 2023 12:16:00 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Income Tax Return and/or BIR Form No. 2316.

We called your mobile number to validate your existing eFOI Request, however, it was unsuccessful.

In order to identify or locate the requested information, we are seeking for further clarification specifically:

1. Did you have a Registered Business between the period 2022 to 2023? Or were you an Individual Earning on Purely Compensation Income from the period 2022 to 2023?

2. If Employed, your EMPLOYER is required to issue a copy of your BIR Form 2316 for Calendar Year/s 2022 to 2023 pursuant to Section 2.83.1 of Revenue Regulations No. 02-98, as amended which provides that “The EMPLOYER shall furnish each employee with the original copies of Form No. 2316 xxx.” As proof that your taxes were withheld. This will serve as your Income Tax Return (ITR).

Kindly coordinate with your employer if you have not receive a copy of your BIR Form 2316 for Calendar Years 2022 to 2023.
