You asked for Replacement for Lost TIN ID.

Response to your request

In reply, kindly proceed to the Revenue District Office (RDO) where you are registered, in your case, you are registered at RDO No. 41-Mandaluyong City.

For further assistance, you may call the Client Support Section (CSS) of RDO 41 at telephone numbers (02) 8531-0691 and (02) 8532-5303.

In case of re-issuance due to damage or lost TIN Card, an Affidavit of Loss and P100.00 replacement fee shall be submitted/paid in the RDO.
