Good day!
I am previously employed in Philippine General Hospital as a nurse from March 1, 2010 to May 31, 2013. I was not able to receive a copy of BIR Form No. 2316 for the year 2012 and 2013. I would like to request a copy of BIR Form No.2316 for January 1, 2012 to May 31, 2013. I need these documents for current employment under the Valenzuela City government. Thank you.

I refer to my letter of May 31, 2021 10:08:06 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR form 2316.
In order for us to work further on your request, we need more information from you regarding your query. Pursuant to regulations during 2012 and 2013, employers are mandated to prepare the BIR Form 2316 and give them to their employees. The employees then attach said form to their Income Tax Returns and file them to the BIR, particularly, those who are not qualified for substituted filing. Hence, your employer, PGH, should have the copy of your BIR Form 2316. In this light, we suggest that you refer to your employer for the document you are requesting.
