May I request a copy of my BIR Form 1902? My employer needed a copy of it and I can't find my copy. Here is my e-mail: ciaavelle@gmail.com

Dear Ms. Gabriel:

I refer to my letter of Aug 15, 2023 05:28:17 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for BIR Form No. 1902.

We called your mobile number today, August 16, to validate your existing eFOI request, however, it was unsuccessful.

Your request for a copy of your BIR Form No. 1902 can be obtained from your employer or from the Revenue District Office (RDO) where your employer is registered. You have to indicate the date of transaction the said form was submitted to them.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Nov 08, 2023 04:39:26 AM, your request will be closed.
