Hi Maam/Sir,

I would like to request a copy of my Form 2316 for year 2022 that is submitted by my employer, Asia Peopleworks, Inc., located at 5/F the Athenaeum Bldg. 160 LP Leviste St Salcedo Village Makati City. This request is made due to the following reasons:
1. To check if the Form 2316 given by the employer to me is the same that is submitted to BIR;
2. A copy of Form 2316 was shown to me later than January 31, 2023;
3. I did not signed yet the Form 2316 that they have given to me.
