I refer to my letter of Jul 12, 2023 10:02:43 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Copy of Income Tax Return (ITR).

In order for us to work further on your request, it would be helpful to know the following details:

1)	What Income Tax Form are you referring to? Is it BIR Form No. 1701 or BIR 
        Form No. 1701A? Please check your Certificate of Registration.
2)     What is the Complete name indicated in the Income Tax Return you filed?
3)	What is your Tax Identification Number (TIN) as indicated in your filed 
        Return?
4)	What Revenue District Office No. did you file your form 1701 or Form 
        1701A?
5)	Did you file online (thru eBIR Forms facility) or over the counter?
