Hi,
My name is Martine Anuel Punzalan. I am a Filipino citizen who is currently in the United States as a permanent resident. I am in the process of purchasing a house here but one of the requirements is my Income Tax Return for the past 2 years. Hence, I am requesting your good office to provide a copy of my 2021 ITR; I already have my 2022 ITR.
In 2021, I worked as a nurse at St. Jude Medical Center for several months. I was not able to get my ITR from them, and I am not sure if I will be able to do that now since I heard that the hospital stopped its operation recently.

I refer to my letter of Jul 14, 2023 08:22:06 AM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for copy of my 2021 Income Tax Return (ITR) and BIR Form 2316.
In reply, can you please provide the Complete or Exact Name of your Employer (hospital) for Calendar Year 2021 and your Employer's Taxpayer Identification Number (TIN).
We inquired in the BIR system and the name of the hospital that you have given does not match our record. And at the same time, you request will be forwarded/endorsed to the Revenue District Office (RDO) where your employer is registered.
Email the requested information at sherry.ann.arcita@bir.gov.ph (as this contains sensitive information) and indicate in the subject email ticket number #BIR-345068314515, to respond.
During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Oct 06, 2023 04:16:58 AM, your request will be closed.
