You may get in touch with Revenue District Office (RDO) No. 40-Cubao at (02) 8863-4000 loc. 4081. This is where your Taxpayer Identification Number (TIN) is registered (and not in RDO 126).

You may also send (via email) your duly accomplished BIR Form 1905 and two (2) government issued identification cards at rdo_40css@bir.gov.ph.
