In reply, you may utilized the Revie’s TIN Inquiry Module which can be accessed on the BIR website at https://www.bir.gov.ph in order for you to know your Revenue District Office (RDO) where your TIN is registered.

Request for a stamped BIR Form No. 1904 shall be made at the concerned RDO.
