May I kindly request for the latest copy of my ITR (Year 2022), BIR Form 1701. I am an insurance agent and would be needing as a requirement to renew my license in the Insurance Commission.

Would really appreciate your help regarding this.

I refer to my letter of Oct 27, 2023 04:53:54 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Request for a copy of ITR BIR Form 1701.

In reply, kindly clarify/provide the following:

1. When did you file your Income Tax Return (BIR Form No. 1701)?
2. Did you file it Online (Thru eBIRForms Facility) or Manually at the Revenue District Office (RDO) where you are registered?
3. What is your Taxpayer Identification Number (TIN)?

Email the requested information at sherry.ann.arcita@bir.gov.ph and indicate in the email subject ticket number #BIR-368721533571, to respond.

During the period of us awaiting your reply, your request will be put on hold. If we do not receive a reply on or before Jan 23, 2024 01:04:35 AM, your request will be closed.
