To Whom It May Concern,

I am writing to request a copy of my BIR 2316 form for the year 2022. My name is Jeremy F. Barcela and my TIN ID is 427-076-805. I was a previous employee of Optum Global Solutions, whose TIN ID is 008-056-783.

I have tried requesting the BIR 2316 form from my previous employer personally and through email, but I have not received any response from them. As I badly need this form for filing my tax annualization, I am hoping for your kind consideration in providing me with a copy of the form.

You requested for a copy of your BIR Form 2316 for the year 2022.

Response to your request:

Based on the reason stated below, we resolve to deny your request as to the following information:   Copy of your ITR for the year 2022.

We regret to inform you that your request cannot be processed because your employer was recently enlisted in this Division and the records are still with its old RDO.  You may request your BIR Form 2316 directly from your employer or you may lodge another request in this portal indicating the old RDO of your employer for proper forwarding and appropriate action.

For your guidance, please refer to the Revised People's Freedom of Information Manual for the Bureau of Internal Revenue, which is available for download at the Bureau's Official Website or www.bir.gov.ph.
